Cigna Dental Care (DHMO)
A network dentist must be selected with this plan. Find a network dentist at www.cigna.com. Most preventative services (cleanings, x-rays) are covered at 100% with frequency limits. Other Services such as fillings, root canals, extractions, crowns, dentures, orthodontic benefits and more are covered at reduced, fixed, preset charges. With the Cigna DHMO, you’ll know exactly what you’ll pay (“copays”) for covered services – even for specialty care with a referral approved for payment. Network dentists are not allowed to charge you more than the co-pay for covered services.
Plan Features:
- No deductibles to pay before you can use your plan
- No annual dollar maximums that limit benefits
- No claim forms to file
- No ID cards are required to receive care
- No age limit on sealants to prevent cavities
- No referrals are required to visit a network orthodontist or for children under seven to visit a pediatric dentist in network
- Orthodontia coverage is available for children and adults
Once enrolled in this plan, employees can view the patient charge schedule for the exact cost. This plan does not have out-of-network coverage.
Cigna Dental Care PPO (Select, Select Mid & Select Plus)
Plan Features:
- You may go to any dentist.
- Cigna PPO network dentists will accept reduced fees for covered services and charge less than an out-of-network dentist.
- There are no waiting periods and all covered services are available on your first day of coverage.
- Orthodontia services and implants are covered under the Select Mid and Select Plus Plans (see chart below).
