On-Site Physicals
The Football Booster Club is sponsoring $15 sports physicals for MAHS student athletes on Wednesday, April 17 after school in the Aux Gym. Students should bring a signed physical form with them to the appointment.
Off-Site Physicals
Piedmont Walton Hospital is happy to partner with Piedmont QuickCare at Walgreens and Piedmont Physicians to provide free sports physicals for high school student athletes in Walton County again this year. Each student athlete must be a current high school student or a rising 9th-12th grade student for the 2024-2025 school year and attend a public or private high school in Walton County. Unfortunately, due to limited availability, we will not be able to accommodate middle school and elementary school student athletes this year.
Please bring your GHSA form with you to your appointment and have the HISTORY portion of the form filled out BEFORE your appointment time to ensure appointment times are kept for the day of the event.
