Football Next Year

Anyone interested in playing football for LMS or LMS next year (2024-2025 season), please complete the following form to get information for upcoming football activities such as: weight training, spring football, summer activities and football next year.

This will put your child on the mailing list for all the mailings for Spring, Summer and Fall activities.

FOOTBALL INTEREST FORM

To practice you must have the following forms:

- Current Physical
- Insurance Waiver
- Concussion Acknowledgement Form

- Sudden Cardiac Arrest

Physical - please be sure your child has a current physical on file or get one before the first day of practice. Please scan the physical with your phone and upload it into our system prior to the first day of practice. Your child will not be able to practice if he does not have a current physical. Walgreens does physicals for like $35. The Urgent Care facility beside Ceilo Blue in the Kroger shopping centerdoes sports physicals also - think they are $25.

Insurance Wiaver, Concussion Awareness and Sudden Cardiac Arrest Forms - all three of these forms are on the LMS Football webapge (Upload Physicals, Insurance Waiver, Concussion Awareness, Sudden Cardia Arrest tab) and can be read and signed online - will need to do this - you do not have to be uploading a physical to do this (you would skip that section)

Eligibility

Student must be eligible academically. All eligible players will have passing averages for 4 out of their 5 classes from the previous semester (in this case, from January of previous school year to May of previous school year). The student has 4 academic classes and the 5th class will be the average of their connection classes. Student can't be repeating a grade (for instance - an 8th grade kid coming from another school wanting to repeat the 8th grade or a kid being held back - you are only eligible your first year in that grade).

I strongly encourage anyone not participating in a sport at the moment to start going to see Robbie Stewart at Uplift Sports Ministries. Not only is he someone that knows what he is doing but his mission is right along the same lines as ours is at LMS Football.
