Please contact us to schedule a registration appointment. A completed registration packet and all required documents must be provided to the school at the time of your appointment.
Required Documents to Register:
Certified Copy Child's Birth Certificate
Copy of Child's Social Security Card
Copy of Enrolling Parent's Driver's License
Immunization Record, Form 3231Certificate of Vision, Hearing and Dental, Form 3300
Proof of Residency - A current utility bill and one of the following are needed - lease/rental agreement, paycheck stub with current address, tax return with current address, current warranty deed or quick claim deed, current homeowner's insurance policy, third person affidavit of residency.
Enrollment packages may be picked up at the school IN ADVANCE
and then brought to your appointment, or you may DOWNLOAD
the enrollment package by visiting the district's registration and enrollment page by clicking the link under "Quick Links."
