MCV4 Vaccine Update - 11th Grade
Per the Georgia Department of Public Health, effective July 1, 2021, children 16 years of age and older, who are entering the 11th grade (including new entrants), must have received one booster dose of the meningococcal conjugate vaccine (MCV4), unless their initial dose was administered on or after their 16th birthday. To help protect children and others from meningitis, this is now a state-required vaccination in addition to the MCV4 vaccine required for rising 7th grade students.

Students can get vaccinations and certificates of immunization from their private physician or at the local health department. Please have your healthcare provider sign off on your child’s immunization form (Georgia Form 3231 Certificate of Immunization) that they have received the MCV4 booster. This form is available at your healthcare provider’s office and will need to be submitted to the main office at your child’s high school prior to the start of school.
