All Student Immunizations must be current and on a GA Form 3231 to start school.
Anyone in need of immunizations, GA Form 3231 updates, transfer out of state immunizations or 3300/4400 form completions
MUST call the Health Department or a Drs. Office for an appointment. PLEASE do not show up without an appointment as you will not be seen.
There will be Back to School Immunization Clinics offered by the Health Department this summer:
7/16/22 from 8:00am-2:00pm @ Loganville HD (Call 770.466.1321 for Appt.)
7/30/22 from 8:00am-2:00pm @ Walton HD (Call 770.207.4125 for Appt.)

Notarized Affidavit of Religious Objection to Immunizations DPH Form 2208
will be accepted in lieu of GA Form 3231
