Monroe Area High School has set up two dates for students to complete their sports physicals! CMS students participating in sports next school year are welcome to attend either event.
All students participating in Walton County School District sponsored sports must have a GHSA physical form on file with their coach. Physical forms can be found under the Eligibility and Forms section of our website. Students can also see their coach for a copy of the form.
1) The first opportunity is April 19, 2023 at Monroe Area High School. Physicals are $15 cash or check made payable to MAHS Touchdown Club. Doctors will be on site beginning at 2:35 PM in the MAHS Fieldhouse. The fieldhouse is located behind the football stadium next to Monroe Elementary. Students must bring the GHSA physical signed by a parent/guardian. Carver students can make arrangements to ride the activity bus from CMS to MAHS. Students need to bring a parent note to the CMS office to get a permission slip to ride the activity bus over to MAHS for tryouts.
2) The second opportunity is April 22, 2023 at Piedmont Walton* between 8:00 AM and 12:30 PM. Physicals are free but you must sign up for an appointment. Click HERE for an appointment. Students must bring the GHSA physical signed by a parent/guardian.
*Physicals on April 22 will be located at Piedmont Walton Hospital's Physicians Building 1 at 2151B West Spring Street, Suite 100.
