All students must have a current Georgia certificate of immunization form 3231, a religious exemption form or a medical exemption to attend school. 

If the required documentation is not received before the first day of 7th grade your child will nit be allowed to attend school. 

The school has sent home 1st and 2nd notice letters if your child does not have an updated form on file. A 3rd and final notice will be sent with the final report card. 

Immunizations are available at most pediatricians offices or any health department in the state of Georgia.
