Immunization Requirements

All students enrolled in school must have a valid 3231 form on file at the school. Georgia law allows for two types of exemptions from the immunization requirements: medical and religious. A 3231 form must be current: either marked complete for school or with an expiration date in the future.
 
All students entering or advancing into 7th grade or transferring from out of state into grades 8th-12th, must have two additional vaccines: Tdap and MCV4 (meningococcal).

Immunization requirements for Pre-K
  • Dtap - 4 doses
  • Polio - 3 doses
  • Hep B - 3 doses
  • Hib - 4 doses
  • MMR - 2 doses
  • Varicella - 2 doses
  • Pneumococcal - 4 doses
  • Hep A - 2 doses
Immunization Requirements for K-12
  • Dtap - 5 doses
  • Polio - 4 doses
  • Hep B - 3 doses
  • MMR - 2 doses
  • Varicella - 2 doses
  • Hep A - 2 doses (born on or after 1-1/2006) 
  • Tdap - Required for students who were born on or after January 1, 2002 and are entering or transferring into 7th grade. As well as all new entrants from out of state entering 8th-12th grades.
  • MCV4 (meningococcal) - Required for students who were born on or after January 1, 2002 and are entering or transferring into 7th grade. As well as all new entrants from out of state entering 8th-12th grades.*Effective July 1, 2021, all students entering or transferring into 11th grade will need proof of a meningococcal booster shot (MCV4), unless their first dose was received on or after their 16th birthday.
