Final Exam Checkout Procedures (Please Read Carefully)

1. Write your name and student ID number at the top of the checkout form. 
2. Turn in the checkout form to your 1st Block teacher by Wednesday, May 23rd. 
3. You must have all required exams (EOC, Finals) complete and your Chromebooks turned in or your teacher will void your permission to checkout.
4. A parent signature is required for students to check out after exams on Thursday, May 24th and Friday, May 25th. If you do not have a returned, signed form, you will not be allowed to checkout. 
5. MAHS cannot accept any calls or notes for students to checkout on exam days. Only approved persons may check a student out of school following normal check-out procedures. 
6. Any class period missed for early checkout will count as an absence. Students will not be permitted to checkout during an exam block. This means that when a student is done with an exam during an exam block they will not be able to check out until that block is over.
7. Students must provide their own transportation if leaving before the end of the school day.
8. Buses will operate on a normal schedule. 
9. After leaving campus, students may not return to school unless there is a scheduled athletic event.
