Bay Creek Elementary School will hold 2018-19 Kindergarten Round-Up April 23 & 24.
Please bring the following items to register:
- Certified Birth Certificate
- Shot/Immunization record on Department of Human Resources Form 3231
- Certificate of Eye, Ear and Dental Examinations Department of Human Resources Form 3300
- Social Security Card or a SSN Waiver Form
- Two Proofs of Residence (current utility bill AND lease/rental agreement)
