Student Services - Immunizations
All students enrolled in school must have a valid 3231 form on file at the school. Georgia law allows for two types of exemptions from the immunization requirements: medical and religious. A 3231 form must be current: either marked complete for school or with an expiration date in the future.
All students entering or advancing into 7th grade or transferring from out of state into grades 8th-12th, must have two additional vaccines: Tdap and MCV4(meningococcal).
Immunization requirements for Pre-K
Dtap- 4 doses
Polio- 3 doses
Hep B-3 doses
Hep A- 2 doses
Immunization Requirements for Kindergarten-12th Grade
Hep B- 3 doses
MMR- 2 doses
Hep A-2 doses(born on or after 1-1/2006)
Tdap- born on or after 1-1-2002 and entering or transferring into 7th grade. All new entrants from out of state entering 8th-12th grades
MCV4 (meningococcal)-born on or after 1-1-2002 and entering or transferring into 7th grade. All new entrants from out of state entering 8th-12th grades.
