To try out for the golf team, you must have the following forms and be eligible:
1. Current Physical (print and get doctor to complete)
2. Insurance Waiver (complete online)
3. Concussion Acknowledgement Form (complete online)
4. Sudden Cardiac Arrest Form (complete online)

Eligibility
A student must be eligible academically in order to try out for the golf team.
All eligible players will have passing averages for 3 out of their 4 classes from the previous semester
(in this case, from August to December).
