It is time make sure your children have received all required immunizations for the 2010-2011 school year!
All students must have a current 3231 form or a religious or medical exemption form on file by the first day of school or they will not be admitted.
In 2007 doses were added to two existing vaccine requirements for all children entering Kindergarten, sixth grade or new entrants (children of any age entering Georgia Schools for the first time or returning to Georgia schools after being out of the system for a year). Parents of rising 6th graders will need to be sure their children are up to date on these vaccines before the 1st day of school:
- MMR (Measles, Mumps and Rubella) – 2nd dose required OR Measles Vaccine (2 doses), Mumps Vaccine (2 doses) and Rubella Vaccine (1 dose) or serologic proof of immunity.
- Varicella (Chickenpox) – 2nd dose required or healthcare provider documentation of disease history or serologic proof of immunity.
The following is a complete list of immunizations that are required for school enrollment.
- DTAP (Diptheria, Tetanus and Pertussis); four to five doses
- Hepatitis B; three doses
- Polio; three to four doses
- MMR (Measles, Mumps and Rubella); two doses
- Varicella (Chickenpox); two doses
- Hib (Haemophilus Influenzae Type b); three to four doses, for daycare and preschool only
- Pneumococcal Conjugate (PCV); for daycare, Head Start and preschool only
- Hepatitis A; for all children born on or after January 1, 2006
The Health department offers the hearing, vision and dental screenings (Form 3300) required by schools, as well as Immunizations and Certificate 3231. Please contact your child’s physician or one of the health department locations listed below for additional information.
